Avera is seeking a Spanish/English speaking Community Health Worker who has lived life experience as an immigrant/refugee and who has navigated the challenges of life in a new setting!

Job Summary
- The Community Health Workers (CHWs) are frontline public health workers who have a close understanding of the community they serve.
- This trusting relationship enables them to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
- This individual would work closely with medical providers, primary care teams, and social service agencies to provide care coordination and connections to improve their health and general well-being through education and provision.
- This individual would work in both clinical and community-based settings.
Education and/or Experience
- High School Diploma or Equivalent required
- A minimum of two years’ direct experience preferred.
- Experience with navigating local medical and social support systems preferred.
- Possible bilingual and medical interpreter qualifications preferred based on specific position needs.
- Possible knowledge and/or experience with certain medical diagnoses preferred based on specific position needs.
- Must be from the community that is being served or have a familiarity of the community.
License/Certification/Registration
- This position requires a valid driver’s license and that, the employee is insurable by the Avera Health automobile liability insurance carrier.
- Must have a Community Health Worker (CHW) Training Certificate from a South Dakota approved program or the ability to obtain within 6-months of hire.
- Must complete the South Dakota Community Health Care Worker Certification within 6-months of hire and maintain certification.
- A Fingerprint Background Check is required for Community Health Worker (CHW) registration.
Work Schedule
- Monday-Friday 8:00am-5:00pm; 80 hours per 2 weeks (flexible hours)
- Occasional weekends and evenings for special events or speaking engagements.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
