HOURS: 40 hours weekly, Monday through Friday, 8 am - 5 pm, with evenings, weekends, and on call as needed.

SALARY: Dependent on experience and qualifications

RESPONSIBILITIES: The CARES Wraparound model is a strengths-based, family driven child abuse prevention and diversion program model designed as a level of care on the front end of the child welfare continuum. The Care Coordinator will implement social work case management to children, youth, and their families by facilitating Family Team Conferencing and the Wraparound process. The Care Coordinator promotes a family driven, youth guided, community based, culturally competent and trauma informed system of care. The position will seek appropriate community resources for the family and monitor the quality of services provided to the family. The Care Coordinator will collect and document required data to ensure the process is outcome-driven and model fidelity is upheld. This position will be a representative of the agency and will develop respectful and professional relationships with clients, referral sources, and community partners. Travel is required.

QUALIFICATIONS: Bachelor’s Degree in social work or related field, plus one to two years of related experience. Licensed Social Worker or Social Work Associate through the South Dakota Board of Social Work Examiners. Must be at least 21 years old. Must successfully complete all agency background checks. Requires valid driver’s license and ability to obtain approved driver status.
