POSITION SUMMARY:
Performs duties involving the planning and implementation of activity programs which meet the physical, spiritual, mental and psychosocial needs and interests of all residents. Participates in the budgeting and planning for new equipment and supplies. Develops the activity care plan for each resident and actively participates in care plan meetings. Will do documentation as required by State and Federal Regulations. Helps create a warm social atmosphere in Bowdle HealthCare. Performs duties of Social Services department in absence of social worker or designee.
REQUIREMENTS AND QUALIFICATIONS:
Education: High School Diploma or Equivalent
License/Certification: Must be qualified or eligible for qualification as Activity Coordinator as required by the South Dakota Department of Health. Must be CNA certified or eligible to become CNA certified through facility.
DUTIES AND RESPONSIBILITIES:
- Demonstrates initiative and judgment in adopting programs and in initiating new activities and techniques for each resident’s liking.
- Coordinate participation of volunteers.
- Maintain adequate recreation equipment and supplies.
- Ability to work well with residents, staff, and volunteers in a leadership capacity and be courteous and cooperative. Must be able to use tact, sympathy, patience, and understanding.
- Willing to work with older people having limited capacities and various ailments. Must take an interest in the resident’s life and take time to listen to them.
- Works with other health care team members in providing and maintaining a holistic health and well-being service to the patient/resident.
- Keeps administration informed of issues in the Activity Department.
- Encourage family participation in resident’s life.
- Attend workshops and keeps informed of any new information, changes affecting the Activities Departments.
- Keeps all information about resident and family confidential.
- Post monthly calendars and bulletin boards. Writes news column monthly as indicated.
- Dresses appropriately for work demand.
- Completes MDS+ Resident Assessment accurately and in timely manner. Develops activity care plan for each resident and participates in care conferences. Documents in resident charts as required by licensure regulating agencies. This includes submission of information to state.
- Exercises general management of the Activities Department. General management is defined as including, but not limiting to: following all laws, regulations, policies, procedures, etc. that apply; maintaining a sound working relationship both within the department and with other departments of the organization; maintaining safety of all and high standards of care with patients; reporting any unusual or irregular activities, actions, situations, etc. to the Administrator; and completing all Activities Dept. supervisor responsibilities assumed at Bowdle HealthCare.
- Maintains a work schedule. Adapts to flexible work hours and other changes and gives notification of absence.
- Accepts responsibility for projects or committees as assigned by the administrator.
- Attends department head, safety, risk management, in-service programs and other meetings or facility functions as necessary.
- Implements, maintains and monitors the department’s QA program in cooperation with the QA Coordinator.
#19-24: In the absence of the social worker or designee:
- Assist in admission, transfer, discharge procedure and have knowledge and understanding of the swing bed program and assessment.
- Completes patient/resident social history, assessment and social service plan of care according to guideline time frames.
- Maintain cumulative progress notes in patient/resident chart and care plan indicating response to treatment and emotional support as required by state and federal regulations.
- Understands social and emotional needs of patient/resident, shows patience and tact, is cheerful and enthusiastic in interaction with resident, family, staff and other persons, holistic care to residents.
- Understands payment sources, private insurance, Medicaid and other to advise and/or assist patient/resident with resources and applications for financial help.
- Assists staff in reviewing contents of Self Determination Act with patient/resident (or significant other), has understanding of advance directives, resources for completion and follows through with processing to appropriate agencies.
- Knows emergency procedures for fire, internal/external disaster plans, code blue, and participates in drills.
- Conforms to established Bowdle HealthCare Center personnel policies.
- Maintains confidentiality of information regarding patient/resident, families, health care personnel, and Bowdle Healthcare Center.
- Responsible for carrying out the directives of the Administrator. If doing so would put the Activities Coordinator in a compromising situation, they shall inform the Administrator in writing and in a timely fashion why the directive cannot be carried out.

LIFT up to 10 lbs.: Frequently required to lift various supplies.
LIFT over 50 lbs.: Occasionally to transfer resident from wheel chair with assistance of others and using transfer belt.
CARRY up to 10 lbs.: Frequently required to lift and carry supplies used for recreational activities.
REACH above shoulder height: Required to reach cupboards or shelves for supplies.
REACH at shoulder height: Required for various jobs directing or demonstrating activity.
REACH below shoulder height: Required to assist resident in wheel chair.
PUSH/PULL: Required to assist people in wheel chairs daily.
Hand Manipulation
Grasping: Required constantly while documenting and while working with various activities.
Handling: Required constantly while documenting and while working with various activities.
Fingering: Required for day to day activities.
Controls & Equipment: May be required to operate an automobile, audio/video equipment, take phone calls and use typewriter and computer.
Other Physical Considerations
Bending: Frequently required while working with seated residents at tables to monitor or assist progress of resident.
Squatting: Frequently required while working with seated residents to speak to or monitor progress.
Kneeling: Frequently required while working with residents seated at tables.
Climbing: Occasionally required to climb stairs in facility or when riding bus.

May be required to walk and stand on carpet, linoleum or cement for extended periods of time.

Sight: Required when observing progress of resident.

OTHER CONSIDERATION AND REQUIREMENTS:
Work Hazards: Must be alert to wet floors when they occur in facility. May be exposed to communicable diseases and physical abuse from residents.
Consequences of Action: Failure to comply with department and/or hospital policies will result in disciplinary action as outlined in the personnel policy manual.
This position requires 30 days notice of resignation.
