Call to Freedom is committed to identifying and responding to the needs of individuals at-risk of human trafficking and sexual exploitation and those working to transition out of human trafficking. The Emergency and Addiction Services Case Manager will specialize in community and rural outreach to establish relationships with service providers and provide emergency services to victims in crisis. Identify and respond to substance abuse treatment needs for clients and identify service providers in communities across the state of South Dakota. Provide CD assessments, consultation, and program development. Key tasks include ongoing outreach to local and rural service partners, CD assessments, state submission and intake/screening processes, crisis response, information and referral to providers, and case management, as necessary.

 

REQUIRED EDUCATION AND/OR EXPERIENCE:

  • Prefer Bachelor’s degree in CD Counseling or Social Work, Counseling and/or experience with victims of violence, trafficking, trauma, and/or substance abuse helpful. 

  • Excellent verbal and written communication skills. Flexible, self-directed, and responsive.

  • This position is hourly FTE, 40 hours/week, must be able to adapt to flexible scheduling; including evenings and weekends as emergencies require.

CERTIFICATION, LICENSURE AND/OR REGISTRATIONS:

  • Certified Addictions Counselor (CAC) or Licensed Associate Counselor (LAC)
  • The ability to obtain and retain a valid driver’s license and motor vehicle insurance.

ESSENTIAL FUNCTIONS AND JOB DUTIES: 

  • Serves as contact for clients in crisis to assist with emergent needs (DV Shelters, homeless shelters).

  • Coordinate responsive and timely service to individuals based upon CTF guidelines.

  • Provide crisis addiction assessments for new and existing clients and assist with state assessment submission and referrals to treatment options as needed.

  • Provide a Mobile Response for clients seeking addiction services in rural areas.

  • Link all clients with crisis intervention and appropriate emergency service providers locally and in rural communities. 

  • Assist outreach team, contractual partners, and CTF Navigators with connecting services in local and statewide communities. 

  • Conduct and coordinate initial screenings and assessments and work with individuals, recognizing needs for safety, autonomy, and confidentiality.

  • Provide case management and coordination of services to outreach clients in a timely manner.

  • Develop and implement individualized case management goals.

  • Develop, collaborate, and communicate with community and rural partners. Continue to strengthen and develop community partnerships in order to meet the diverse needs of clients.

  • Maintain documentation and records as appropriate.

  • Provide training to community partners, service providers, public groups, schools, etc., as requested to increase awareness, identification, reporting and referral of victims for services.

  • Be available for flexible hours to provide crisis services, including weekends and evenings as needed.

  • Ensure program operations and activities adhere to legal guidelines and internal policies.

  • Reliable transportation for client needs when company vehicle is unavailable or coordinate long distance transportation through CTF. 

  • Other duties as assigned.
