Join Our Team as a Family Development Case Manager

If so, we have an exciting opportunity for you to join our team as a Family Development Case Manager at Call to Freedom in Sioux Falls, SD.

Call to Freedom is committed to identifying and responding to the needs of individuals at-risk of human trafficking and/or exploitation and those working to transition out of the life of human trafficking. The Family Development Case Manager’s purpose is to identify needs, implement strategies, and coordinate individualized services for victims and survivors. Key tasks include advocating and educating families that are supported by Call to Freedom in order to cultivate positive, healthy family relationships. 

REQUIRED EDUCATION AND/OR EXPERIENCE:

  • Prefer Bachelor’s degree in Social Work or Child/Human Development or related field experience. Experience in case management with victims of violence, trafficking, trauma, and/or substance abuse essential.
  • Training in trauma-informed care beneficial. 
  • Achieve SOAR certification and be primary contact for agency and clients.
  • Achieve certification in Love and Logic curriculum.
  • Excellent verbal and written communication skills. Flexible, self-directed, and responsive.
  • Approximately 40 hours/week, must be able to adapt to flexible scheduling; including evening and weekend hours as needed.

CERTIFICATION, LICENSURE AND/OR REGISTRATIONS: 

  • The ability to obtain and retain a valid driver’s license and proof of motor vehicle insurance.

ESSENTIAL FUNCTIONS AND JOB DUTIES: 

  • Be an advocate for assigned clients and their children. Provide responsive and timely service to individuals.
  • Work with Marissa’s House and Outreach clients to develop and implement individualized family development and financial goals and plans.
  • Observe interactions between clients and their families, therapists, or other treatment providers to assess progress.
  • Maintain documentation and records as appropriate.
  • Provide supportive services and case management to help clients deal with barriers related to their families or children.
  • Provide supportive services and case management to help clients with financial barriers.
  • Develop community based financial program for clients to participate in and to receive ongoing support by developing individualized goals and plans. 
  • Coordinating with other professionals such as social workers, psychiatrists, school personnel, medical providers, or attorneys to ensure that each client’s needs are met by all parties involved with their reunification or development plan. 
  • Helping clients apply and obtain services from government agencies such as the Department of Social Services and/or ensuring families are connected with local resources.
  • Develop and foster relationships with local and state childcare providers to ensure clients have reliable and safe childcare.
  • Implement Love & Logic curriculum to Marissa’s House and Outreach clients.
  • Implement Budgeting curriculum to Marissa’s House and Outreach clients in order to teach daily, weekly, and monthly budgets. 
  • Collect and track data on client progress over time to determine whether goals have been met and the services that have been provided.
  • Conduct home visits to observe family interactions firsthand and meet with parents privately to discuss issues.
  • Coordinate with court systems to ensure clients attend scheduled hearings and assist in filling out or submitting necessary paperwork.
  • Implement occupational therapy plan formed with the Occupational Therapist to carry out individual client goals including helping schedule and conduct family-centered education groups.
  • Provide education to the community.
  • Other duties as assigned.
