Amount of Travel Required: Travel to family homes in the area and some possible travel for in-state or out-of-state training
Work Schedule: Full-time position requiring flexible schedule including some evenings and weekends
POSITION SUMMARY
The Family Support/Shared Living Coordinator position works closely with participants, families/guardians, host families and community resources to provide the tools, training and knowledge needed so individuals receive quality services.
This role requires adherence to all policies and procedures of Volunteers of America, Dakotas and Administrative Rules of South Dakota (ARSD) while treating everyone with dignity and respect. ESSENTIAL FUNCTIONS
Family Support
A. Work cooperatively with the fiscal agent and the Division of Developmental Disabilities to coordinate a program of family supports in the city of Sioux Falls.
B. Administer the program by networking with other community resources, developing resources that are not available, and offering families choices to meet their needs.
C. Facilitate a needs assessment and develop an Individual Service plan documenting the needs and services provided to each family.
D. Monitor and document all program expenditures including administrative costs, service coordination and financial assistance provided to each family.
Shared Living
A. Responsible to educate, facilitate, coordinate and monitor shared living providers in accordance with agency policy and procedures
B. Perform administrative functions to ensure quality and service standards are met
C. Maintain ongoing communication with participants, host home providers, conflict free case managers, parents/guardians and the Division of Developmental Disabilities to coordinate services
D. Conducts face to face visits each month with people living in host homes, and completes required site documentation for each visit
E. Facilitates interviewing/contracting for Shared Living Providers
F. Participates in evaluation process of vetting all host home families
G. Orientates, coordinates and trains Shared Living Providers
H. Monitors completion of mandatory trainings and assessments of shared living providers
I. Provides oversight and maintenance of participants records to ensure that data is current and comprehensive
- Reinforces paperwork expectations with host home providers and relief staff
J. Monitors participants for safety, including assurance that each home environment is safe, clean and in good repair
K. Reviews the host home documentation to ensure completion and accuracy (e.g. progress notes, training plans, daily notes, emergency drills, Medication Administration Records (MARs), etc.)
L. Communicate with HR regularly on Shared Living Independent Contractors and their family members regarding background checks needed
Other
A. Meet all requirements set by the Administrative Rules of South Dakota, including training requirements not limited to Med-Aide Certified
Responsible for other duties as assigned.
POSITIONS QUALIFICATIONS SKILLS & ABILITIES
Education: Degree in human services, social work, sociology, psychology, or related field
Experience: Knowledge of existing services and prior experience working with individuals with intellectual disabilities, or a family member of a person with a developmental disability.
Skills: Ability to work independently with minimal supervision once training is completed. Excellent verbal/written communication skills and good computer skills required.
Licenses: Must possess a valid driver’s license and provide proof of insurance with a clean driving record to drive agency vehicles.
