Southeastern Directions for Life is seeking an organized and team-oriented individual for an IMPACT Case Manager position working with adults who have severe mental illness (SMI) in an Assertive Community Treatment (ACT) program at our 5th Street location, Community Support Services. JOB SUMMARY: You will be part of a team that is responsible for assisting individuals with a mental illness through their recovery process. Case managers in this position provide direct service, assessment, and service coordination to our clients. Staff will be trained on medication administration and assist clients with medication management, life skills development, and recovery. The IMPACT Case Manager must have a strong commitment to the rights and abilities of those served and be able to establish supportive trusting relationships. Position does require being part of a rotating on-call system.
Work schedule is 10 hour shifts; Wednesday-Saturday or Sunday-Wednesday.
QUALIFICATIONS: Associate's degree in a social science or human services field, Bachelor's degree preferred. At least one year experience providing services to individuals with severe mental illness (SMI), including those with co-occuring substance use disorders is preferred. Must have a current driver's license, car insurance, and a driving record acceptable to Southeastern’s insurance provider. Must pass a comprehensive criminal background check to include fingerprinting, Dept. of Social Services Central Registry check, Motor Vehicle background check, and drug screening.
