Job Description

$15,000 Sign-On Bonus for 3 Year Commitment*

 

Bowdle Healthcare Center Nursing Home is a maximum of 37 bed facility. We are attached to our hospital and clinic for easy access to medical and specialty services for residents. We have an in-house provider available at all times.

POSITION SUMMARY:

Supervises planning and administration of all nursing care, to provide and maintain high quality resident care.  Responsible for organization, management, guidance, and supervision of Nursing Service Department and nursing facility guidance and supervision and assist with administrative decisions in the absence of the administrator.  Will ensure the safety and overall well-being of all residents. 

 

REQUIREMENTS AND QUALIFICATIONS:

Education: Graduate from an accredited school of nursing.

License/Certification: By State of South Dakota Board of Nursing as a Registered Nurse.

Experience: Three years of nursing experience, including satisfactory experience as charge nurse during which executive ability is demonstrated.

 

DUTIES AND RESPONSIBILITIES:      

Assume responsibility for coordinating delivery of all nursing care by working closely with nursing staff to facilitate resolution of any problems regarding the planning or delivery of total resident care.

 

Assumes responsibility to interview, hire, and orientate new employees in nursing; writing and delivery of constructive employee evaluations; provision of necessary guidance and/or instruction for performance deficiencies noted; and scheduling of nursing staff to provide optimum coverage to maintain established standards of nursing care as reasonably and economically as possible.

 

Visits residents regarding condition and quality of care, maintain a schedule for recertification, and coordinate discharge planning needs in conjunction with medical and ancillary staff, as well as home care services.

 

Keeps abreast of current trends in nursing practice and nursing administration through review of professional literature, attendance at pertinent workshops and in-service programs, professional memberships, etc.

 

Consults with the administrator regarding administrative aspects of the nursing department and all departments in the nursing facility; also reports any unusual incidents.

 

Assists in planning and budgeting process for the nursing department. Establishes charges for services as well as maintaining cost controls.

 

Assists in requisitioning, storing, distribution of equipment and supplies for nursing service and reports the need for repair of equipment as needed for services as necessary.

 

Have knowledge and support of the philosophy, policies, and standards of the nursing home and assume responsibility for the development and maintenance of all policies and standards of the nursing department including providing means to assure their explanation and implementation.

 

Promotes harmonious relationship among nursing personnel, with the medical staff, the residents, the public, and other departments.

 

Supervise maintenance of all resident medical records of nursing care and medical treatments executed by nurses and proper use of all nursing facility forms as required.

 

Assure adequate planning of in-service programs to keep the nursing staff updated in all aspects of the nursing process and current nursing practice and according to OBRA guidelines.

 

Have the ability to assume the leadership role effectively in a major emergency or disaster, including knowledge and expedient execution of disaster plan.

 

Be available to assist nursing staff with direct resident care.

 

Have sound knowledge of the legal aspects of nursing through appropriate assignment of nursing responsibilities and have thorough teaching and supervision of staff.  Coordinate NA training as outlined by DOH and maintain a qualified trainer.

 

Serves as coordinator of discharge planning, infection control, and PERTS incident reporting system and staff and resident immunization programs.

 

Attends department head, medical staff, safety, risk management, in-service programs, and other meetings as necessary.

 

Be responsible for projects and/or committee work as assigned by the administrator; assisting in administration decisions in absence of administration.

 

Delegates the responsibilities of DON to charge nurse in the event of DON absence.

 

Schedules Charge/Floor Nurse to ensure coverage at all times.  Coordinates the scheduling of Nursing Assistants.  Stays informed of Activities and Social Services schedules and makes recommendations when needed.

 

Maintain personnel files having current documentation of renewed licenses and certification.

 

Accountable for the classification of each resident according to the outcome of the MDS+ and coordinating and conducting care conferences and developing RAPS and care plans.

 

Exercises general management of the nursing department and nursing facility.  General management is defined as including, but not limiting to:  following all laws, regulations, policies, procedures, etc. that apply; maintaining a sound working relationship both within the nursing department and nursing facility and with other departments of the organization; maintaining safety of all and high standards of care with residents; reporting any unusual or irregular activities, actions, situations, etc. to the Administrator; and completing all DON responsibilities assumed at Bowdle Nursing Home.

 

Responsible for carrying out the directives of the Administrator.  If doing so would put the DON in a compromising situation, the DON shall inform the Administrator in writing and in a timely fashion why the directive cannot be carried out.

 

PHYSICAL ACTIVITY REQUIREMENTS:

Primary Physical Requirements

LIFT up to 10 lbs.: Frequently when lifting charts, supplies, etc.

LIFT 11 to 25 lbs.: Occasionally when lifting equipment.

Lift 26 to 50 lbs.: Occasionally when lifting and transferring residents, equipment, etc.

Lift over 50 lbs.: Occasionally when lifting and transferring resident with assistance of another staff member.

CARRY up to 10 lbs.: Frequently when lifting charts, supplies, etc.

CARRY 11 to 25 lbs.: Occasionally when lifting equipment.

CARRY 26 to 50 lbs.: Occasionally when lifting and transferring residents, equipment, etc.

CARRY over 50 lbs.: Occasionally when lifting and transferring resident with assistance of another staff member.

REACH above shoulder height: Necessary in performing daily routine resident care.

REACH at shoulder height: Necessary in performing daily routine resident care.

REACH below shoulder height: Necessary in performing daily routine resident care.

PUSH/PULL: Occasionally with equipment used.

 

Hand Manipulation

Grasping: Frequently necessary in performing various resident care activities, especially use of pen when documenting into medical record.

Handling: Frequently with use of equipment and use of pen in documentation.

Fingering: Daily performance of duties such as various procedures, charting, and giving meds.

Controls & Equipment: Blood pressure gauge, thermometer, stethoscope, wheelchair, cardiac monitor, IV feeding pumps, NG gastric and urinary catheters, defibrillator, oximeter, glucometer, suction machine, telephone, paging system, Hoyer lift, whirlpool, transfer belts, walkers, cane and computer.

 

Other Physical Considerations

Twisting: Necessary with various resident care activities.

Bending: Necessary with various resident care activities.

Squatting: Necessary with various resident care activities.

Kneeling: Necessary with various resident care activities.

Crouching: Necessary with various resident care activities.

Talking: Very necessary in daily communication with resident, families, facility staff and public.

Hearing: Very necessary in daily communication with resident, families, facility staff and public.

Sight: Necessary in performing duties effectively and correctly.

Smelling: Required for accurate maintenance and detection of wounds, urinary tract problems, etc.

Tasks and procedures performed by employee involve risks classified by CDC as:

Category I   (Direct contact with blood or other bodily fluids to which universal precautions apply).

Refer to Exposure Control Plan for additional information.

 

OTHER CONSIDERATION AND REQUIREMENTS:

Work Hazards: Risk of being exposed to communicable or infectious disease and back strain if proper safety precautions are not followed.

Consequences of Action: Failure to comply with department and/or facility policies will result in disciplinary action as outlined in the personnel policy manual.

This position requires 30 days’ notice of resignation.
