Position Description
Oversees the agency’s Medication Clinic total operations and community nursing functions under the CARE Program, while adhering to the agency’s mission and values, and creating a culture of continuous improvement and teamwork.
Duties / Responsibilities:
Coordinates the med clinic operations to ensure proficiency and quality service is delivered.
Engages with agency leadership team and other departments on promotion of continuity of care and operations.
Manages contract physician/CNPs schedules and day-to-day needs.
Manages med clinic nursing staff including oversite of personnel needs and ensures accountability in timely completion of duties.
Manages psychiatric clinic referrals.
Oversees the Health Home program.
Responsible for day-to-day scheduling of med clinic staff, medication pass schedule at the CARE Center, and coordination and scheduling of client/patient visits.
Completes comprehensive client/patient assessment and formulates or updates nursing care plans.
Conducts group and individual activities appropriate to treatment plans when needed.
Provides comprehensive patient care according to the individual’s treatment plan.
Assesses the physical needs of clients/patients, observes clients/patients for illness or injury, and answers questions of staff regarding behavior of patients.
Administers medications and treatments as prescribed by a physician/CNP to ensure client/patient medical needs are addressed and side effects from medications and treatments are documented.
Monitors client/patient response to medications and treatments and relays data to the physician/CNP.
Explains prescribed medications and treatments to clients/patients and Clinical Specialists and other staff as needed.
Provides input during treatment team meetings to develop a comprehensive treatment plan for each client/patient.
Ensures compliance with all State, Federal and agency regulations, and standards.
Ensures medication destruction is complete according to state requirements.
Maintains and schedules biohazard destruction compliant with federal regulations.
Maintains proper records for all medications within the facility according to state requirements.
Ensures accuracy of physician/CNP documentation and orders.
Completes and ensures accuracy of all medication refills and provider orders within scope of practice.
Coordinates with SUD staff for MOUD treatment plans and appointments.
Provides Medication Aide training, certification, and monitoring.
Maintains and assist with client/patient medication assistance applications.
Reviews adult group home applicants.
Other duties as assigned.

EHR and E-Scripts knowledge.
Education and Experience:
Registered Nurse (RN) in South Dakota
CPR/AED/BLS certification required
Experience in mental health preferred
Two years prior supervisory experience preferred but not required
Physical Requirements:
Exposure to infectious diseases.
Exposure to occupational dangers like accidental needle pricks.
Requires long hours of standing and moving.
